The Ortho Evra® birth control patch has been linked to an increased blood clot risk. This page includes information for birth control patch users about deep vein thrombosis in the leg, pulmonary embolism in the lung, and transient ischemic attack (TIA) in the brain.
All combination hormonal contraceptive users, including women using the Ortho Evra® birth control patch, have an increased blood clot risk. However, the degree of risk may be much greater for Patch users than Pill users. In November 2005, the FDA approved a warning to the Ortho Evra® label, revealing that Patch users are exposed to 60 percent more estrogen than Pill users. A high level of estrogen in the body is linked to the formation of blood clots.
Prior to the addition of the new Ortho Evra® label, the makers of the patch claimed that the degree of risk for Ortho Evra® users was the same as the risk faced by women who use the pill. However, even at that time, Douglas & London and others recognized that a frighteningly high number of women were experiencing health problems while using the patch. A formal warning did not reach the public until many months after it was evident that the Patch posed more of a health threat than the Pill.

Deep vein thrombosis occurs when a blood clot, also known as a thrombus, develops in a person’s deep veins – most often in the leg. The veins are located near the bones and are surrounded by muscle. A deep vein thrombus is dangerous because it can break loose and travel through the bloodstream to the lung, causing a life-threatening pulmonary embolism.
Using the Ortho Evra® birth control patch is just one cause of deep vein thrombosis of the leg. Other causes include injury, prolonged inactivity, or a blood-clotting abnormality.
Unfortunately, a person with deep vein thrombosis may not have any symptoms. When symptoms are present, they usually include pain, tenderness, or swelling.

Pulmonary embolism is a sudden blockage in a lung artery. Blood clots that form in the deep veins of the legs (deep vein thrombosis) are the most common cause of pulmonary embolism, but clots can form anywhere. The clots break free from a leg vein and travel to the pulmonary artery in the lung, blocking blood flow.

A transient ischemic attack, also referred to as TIA, occurs when a blood clot temporarily blocks blood flow to a part of the brain. The condition has the same symptoms as a stroke. However, unlike a stroke, a TIA only lasts about 10 to 20 minutes or less. After that time, blood flow returns and symptoms disappear.
Since TIA is considered a warning sign of stroke, it is important that people who have experienced the above symptoms visit a doctor right away. If someone is displaying the above symptoms, seek emergency help immediately as he or she may be having a stroke.
